The NH Long-Term Care Summit brought together the various provider groups, advocacy organizations, regulatory and payer representatives who lead NH’s long-term care system for a focused public policy conference. The Summit featured speakers and panel discussions that addressed the needs and challenges for long-term care policy, explore innovative solutions, and craft a working policy agenda.

This session will explore the challenges faced by medically-cleared patients in transitioning out of the hospital, particularly when their discharge hinges on securing nursing facility level of care or community based in-home supports. The number of care options and costs can leave patients and caregivers overwhelmed, leading to delays in hospital discharge and recovery. Attendees will engage in interactive solution-mapping that fosters collaboration between the many systems of care for healthy aging stakeholders.
